PINKNIFICENT Cold Cap Service Agreement

Thank you for choosing PINKNIFICENT. It is our pleasure to support you throughout your cold cap therapy journey.

Once all parties have agreed to the quoted services, a deposit is due immediately to secure your service date(s) and appointment. Services are not considered booked until the required deposit has been received.

Treatment Scheduling

Once your treatment dates have been confirmed, please contact your infusion center to verify your chemotherapy appointment time. This allows your Capper to schedule the appropriate arrival time and ensure your cold cap therapy begins according to protocol.

Grant Recipients

For clients receiving assistance through the Cap & Conquer Grant Program, please note that the grant does not cover travel or parking expenses. A travel fee will apply for appointments located more than 10 miles from the designated service area, and any applicable parking fees are the client’s responsibility.

Once your service dates have been confirmed, this agreement becomes effective.
